How To Prepare For Your Visit
It’s important to gather any medical records you may have in order to help your doctor get a better understanding of your past health history. Be sure to bring a progress letter from your physical therapist if you have one, as well as an operation report if you have received surgery through another provider. It’s also a good idea to let your doctor know about any and all medications you are taking at the moment.
Don’t forget to bring your ID and insurance card! Having these items ready will ensure you have a swift and smooth visit at OCC.
Prior to your appointment, be sure to fill out any pending new patient forms online links to Financing page. If you’d like to complete them at our office, we recommend arriving 15-30 minutes before your scheduled appointment.
